TERMS AND CONDITIONS

The undersigned, acting in their own name and/or in their capacity as holders of parental responsibility over the minors indicated in the registration form, hereby declare that, by means of this document, they are proceeding with the registration of all the individuals specifically named therein, without any exclusion, for the events entitled “CALCIO LANCETTI”, to be held at spazioSERRA, managed by spazioSERRA – Associazione di Promozione Sociale (Registered office: Via Matteo Maria Boiardo 20 – 20127 Milan; Operational headquarters: Lancetti Station of the Milan Passante Ferroviario; Tax Code 97930140153), on the dates from 15.02.2025 to 01.03.2025, inclusive.

This declaration and registration shall be deemed to refer, without distinction and in its entirety, to each of the individuals indicated in the form, producing effects with respect to all of them, without exceptions or limitations.
 

DECLARE AND AGREE 
 

1. To participate as players in the free, public football tournament organized by the collective “Fidaty s.p.a.” (composed of members Zelano Edoardo, Girelli Giulia, Croin Pietro), held from 15.02.2025 to 01.03.2025, at spazioSERRA (managed by spazioSERRA – Social Promotion Association; Registered office: Via Matteo Maria Boiardo 20 – 20127 Milan; Operational headquarters: Lancetti Station of the Milan Railway Passante; CF 97930140153).

2. To be fully aware that participation in the activities is voluntary, and that any action carried out during the activities is strictly voluntary and optional.

3. To assume personal responsibility for any consequences arising from such actions, both civilly and criminally.

4. To be fully aware of the potential risks involved during the activities offered, including, by way of example and not limitation, bruises, sprains, fractures, and other injuries.

5. To release the organizer, its representatives, volunteers, and collaborators from any liability for physical, material, or moral damages that may arise from their participation in the event.

6. To declare that they are in good health and that have no medical conditions that could hinder their participation in the tournament.

7. To authorize the organization and third parties authorized by it, for institutional purposes only, to photograph and film me, to record their voice, and to transmit, publish, and use any recordings or images without requesting any compensation.

8. To authorize the organization to process their personal data solely for purposes related to the above-mentioned activities, in compliance with principles of fairness and lawfulness.

9. To give written consent to the use of any electronic contact details provided for the purpose of receiving promotions of the organization’s events, as well as general and safety-related communications.

10. To confirm that they have read and understood these terms and conditions and fully accept them.
     

As a result of the above, the undersigned parties release the organization, the associations, and their governing bodies from any liability that may arise as a consequence of participation in the activities, for any damage suffered personally or caused to third parties during the performance of such activities.

For participants under the age of 18, this release must be mandatorily completed and signed by a parent or legal guardian.
